Job Summary
A company is looking for a Population Health Specialist.
Key Responsibilities:
- Supports Population Health Manager in implementing provider engagement initiatives at a practice or clinic level
- Participates in coaching and management on pre-visit planning techniques and closure of gaps in care
- Establishes strong personal relationships with Accountable Care Teams and stays informed about the industry and policy landscape
Required Qualifications:
- Bachelor's degree in Healthcare Administration, Business Administration or related field
- 1+ years of healthcare experience or the knowledge, skills, and abilities to succeed in the role
- Ability to operate in a fast-paced and dynamic environment
- Strong organization skills and detail-oriented
- Knowledge of the U.S. healthcare system
