Job Summary

A company is looking for a Population Health Specialist.

Key Responsibilities:

• Supports Population Health Manager in implementing provider engagement initiatives at a practice or clinic level
• Participates in coaching and management on pre-visit planning techniques and closure of gaps in care
• Establishes strong personal relationships with Accountable Care Teams and stays informed about the industry and policy landscape

Required Qualifications:

• Bachelor's degree in Healthcare Administration, Business Administration or related field
• 1+ years of healthcare experience or the knowledge, skills, and abilities to succeed in the role
• Ability to operate in a fast-paced and dynamic environment
• Strong organization skills and detail-oriented
• Knowledge of the U.S. healthcare system
