Population Health Strategy Associate

Job Summary

A company is looking for a Population Health Strategy & Ops Associate to play a critical role in defining and implementing value-based care strategies for underserved communities.

Key Responsibilities

• Define value-based care roadmap in collaboration with the Chief Medical Officer and Population Health team
• Design, implement, monitor, evaluate, and improve health programs to deliver impactful results
• Collaborate with cross-functional teams to launch and scale new health programs

Required Qualifications

• 2+ years of experience in program or product management and 1+ year of experience with an early-stage startup
• Experience with products that automate or streamline complex workflows, or experience with a digital health startup is a plus
• Experience conducting quantitative analysis and qualitative research to inform strategic decisions
• Ownership mindset with a strong desire to work in a fast-paced, early-stage startup environment
• Passion for helping individuals experiencing complex chronic needs such as homelessness, severe mental illness, and substance use disorder
