Job Summary

A company is looking for a Senior Population Health Strategy Specialist.

Key Responsibilities

• Develops and designs complex population health strategy initiatives and operational plans to enhance care coordination and health outcomes
• Monitors care management trends and analytics to support program initiatives and identify high-risk member populations
• Tracks program participation and member outcomes to assess program efficacy and make recommendations for improvement

Required Qualifications, Training, and Education

• Requires a Bachelor's degree
• 4 - 6 years of related experience
• Equivalent experience may be considered based on applicable knowledge and skills
• Must be authorized to work in the U.S. without the need for employment-based visa sponsorship
• Candidate must reside in Georgia or South Carolina
