Job Summary

A company is looking for a Practice Transformation Manager to lead the implementation of value-based care programs with provider practices.

Required Qualifications

• 3+ years of experience in practice transformation or healthcare operations
• Direct experience in a value-based care environment with physician practices
• Strong understanding of value-based care models and primary care operations
• Familiarity with EMR systems and population health management platforms preferred
