Practice Transformation Manager

Job Summary

To lead the implementation of value-based care programs, the full-time remote Practice Transformation Manager will serve as the primary contact for provider practices, driving adoption of population health management strategies and quality performance initiatives to improve patient outcomes.

Key responsibilities

• Lead the deployment of value-based care models and optimize clinical workflows in collaboration with providers and practice staff
• Develop population health management programs and facilitate care coordination to enhance patient engagement and quality performance
• Build strong relationships with practice administrators and internal teams to drive collaboration and sustainable change

Required qualifications

• Bachelor's degree in Healthcare Administration, Public Health, Nursing, or a related field
• 3+ years of experience in practice transformation, care coordination, or population health in a healthcare operations role
• Direct experience with physician practices or provider organizations in a value-based care environment
• Strong understanding of value-based care models and primary care practice operations
• Ability to manage multiple practice relationships while delivering measurable results
