Practice Transformation Specialist

Job Summary

Passionate about improving patient care, the full-time remote Practice Transformation Specialist will implement ACO initiatives, support wellness visits, and train healthcare staff while directly engaging with providers to enhance practice performance.

Key responsibilities

• Implement products and execute ACO initiatives while ensuring practices meet goals outlined in Practice Transformation Plans
• Assist in training office staff and providers on processes and technologies, while assessing training needs and fostering relationships with healthcare executives
• Listen to practice concerns and relay information to management to identify opportunities for improvement

Required qualifications

• 4-6 years of relevant work experience in medical practice management, with significant administrative experience in a clinic setting accepted in lieu of educational requirements
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
