Job Summary

A company is looking for a Pre-Visit Care Connector to work remotely in multiple locations.

Key Responsibilities
  • Conduct telephonic outreach to members and assist with Pre-Visit Assessments
  • Document demographic and medical history information accurately
  • Provide excellent customer service and maintain compliance with HIPAA regulations
Required Qualifications
  • High School Diploma/GED or equivalent experience
  • At least 2 years of call center or telephonic customer service experience, preferably in healthcare
  • 2+ years of healthcare experience or social work/community outreach experience
  • 1+ years of experience in collecting and documenting medical history information
  • Proficient in Microsoft Word, Excel, and Outlook
