Job Summary
A company is looking for a Pre-Visit Care Connector to work remotely in multiple locations.
Key Responsibilities
- Conduct telephonic outreach to members and assist with Pre-Visit Assessments
- Document demographic and medical history information accurately
- Provide excellent customer service and maintain compliance with HIPAA regulations
Required Qualifications
- High School Diploma/GED or equivalent experience
- At least 2 years of call center or telephonic customer service experience, preferably in healthcare
- 2+ years of healthcare experience or social work/community outreach experience
- 1+ years of experience in collecting and documenting medical history information
- Proficient in Microsoft Word, Excel, and Outlook
