Job Summary

A company is looking for a Pre-Visit Care Connector.

Key Responsibilities
  • Conduct telephonic outreach to members and receive inbound calls
  • Complete Pre-Visit Assessments by documenting member information and identifying care gaps
  • Provide excellent customer service while meeting quality and productivity metrics
Required Qualifications
  • High School Diploma / GED or equivalent work experience
  • 2+ years of call center and/or telephonic customer service experience, preferably in healthcare
  • 1+ years of experience collecting and documenting medical history information
  • Proficient in Microsoft Word, Excel, and Outlook
  • Ability to work during normal business hours with flexibility for overtime or weekends
