Job Summary

A company is looking for a Pre - Visit Care Connector.

Key Responsibilities

• Conduct telephonic outreach to members and receive inbound calls
• Document demographic and medical history information accurately during assessments
• Ensure members have access to Primary Care Providers and identify gaps in care

Required Qualifications

• High School Diploma / GED or equivalent work experience
• At least 2 years of call center or telephonic customer service experience, preferably in healthcare
• 2+ years of healthcare or social work/community outreach experience
• 1+ years of experience collecting and documenting medical history information
• Proficient in Microsoft Word, Excel, and Outlook
