Job Summary

Seeking a strategic and experienced Product Lead, Chronic Care Management, this full-time remote role will manage the product strategy and roadmap for chronic condition management, focusing on driving impactful behavior change and simplifying workflows for patients and clinicians.

Key responsibilities

• Own the product strategy and roadmap for chronic condition management, setting direction and operating with significant autonomy
• Lead complex initiatives from discovery through delivery, including clinical program design and medication access workflows
• Mentor and elevate the product management team, providing structured feedback and fostering a high-performance team environment

Required qualifications

• 8+ years of product management experience with a track record of operating at a senior level
• Domain experience in chronic condition management and familiarity with medication access dynamics
• Proven ability to define ambiguous problem spaces and build consensus across diverse stakeholders
• Strong product craft with the ability to write strategy documents and executive narratives
• Experience with Agile development methodologies and tools such as JIRA and Confluence
