Job Summary

To support an integrated care model, the full-time remote Psychiatric Nurse Practitioner - CT, FL, NY, OH will manage behavioral health evaluations and collaborate with primary care providers in long-term care settings, focusing on patient goals and care coordination.

Key responsibilities:

• Manage behavioral conditions in collaboration with social workers, psychiatrists, and medical providers
• Perform comprehensive behavioral health assessments and document findings per regulations
• Facilitate and evaluate the implementation of the member's plan of care, adjusting as necessary

Required qualifications:

• Current, unrestricted Nurse Practitioner license in Connecticut, Ohio, Florida, or New York (or ability to obtain)
• Ability to obtain DEA licensure/Prescriptive Authority post-hire in relevant states
• National PMHNP Certification or ability to obtain
• Post-graduate hands-on clinical experience
