Job Summary
Supporting East Coast hours in a fully remote capacity, the Qualified Professional Intake Coordinator will serve as the first point of contact for individuals and families entering services, managing referrals, collecting intake information, and ensuring a smooth transition to clinical services.
Key Responsibilities
- Respond promptly to questions and referrals from assigned counties, managing multiple referrals as needed
- Collect and review intake, referral, and admission information to assess service appropriateness and urgency
- Maintain regular communication with referral sources to support timely intake progression
Required Qualifications
- Credentialed or credential-eligible as a Qualified Professional (QP)
- Master's degree in a related human services field with a minimum of 1 year of full-time IDD experience, or a bachelor's degree in a related field with 2 years of experience, or an unrelated field with 4 years of experience
- Experience providing professional support to individuals with Intellectual and Developmental Disabilities
- Highly organized with the ability to manage competing priorities in a fast-paced environment
- Collaborative team player with strong interpersonal skills
