Quality Care Coordinator

Job Summary

A company is looking for a Quality Care Management Coordinator.

Key Responsibilities

• Improve health outcomes of patients through proactive collaborative care
• Address preventive health measure outcomes and gaps in care related to HEDIS and HIV/AIDS Bureau measures
• Engage in telephonic interactions with patients to enhance care and retention

Required Qualifications

• High School Diploma/GED required; Associate or Bachelor's Degree preferred
• Bilingual in Spanish preferred
• Strong knowledge of Healthcare Effectiveness Data and Information Set (HEDIS)
• Understanding of HIV/AIDS Bureau Ryan White Standards
• Ability to work in both remote and field settings
