Job Summary
To support quality outreach initiatives, the part-time Quality Outreach Coordinator will conduct clinical outreach calls, manage member records, and assist in closing care gaps for high-risk members in a completely remote environment.
Key responsibilities
- Conduct incoming and outgoing calls to educate members on closing gaps in care and provide health education materials
- Maintain accurate records of outreach efforts, member issues, and barriers to care
- Act as a liaison between members, providers, and community resources to facilitate appointment scheduling and support
Required qualifications
- Associate's degree in a healthcare field or 3 years of relevant work experience
- 2 years of experience in case management, community health, or a health insurance environment
- Minimum of 2 years of experience working with members to close gaps in care
- 1 year of managed care experience
- Knowledge of the Managed Care or Health Insurance Industry and medical terminology
