Radiology Care Coordination Specialist

Job Summary

To support patient care coordination, the full-time Radiology Care Coordination Specialist will manage scheduling and referrals for specialty providers, track patient interventions and outcomes, and serve as a resource for patients and families, all while working remotely.

Key responsibilities

• Coordinates patient scheduling and referrals to specialty providers, ensuring clear communication and follow-up
• Tracks and documents patient interventions and outcomes, providing timely updates to care providers
• Collaborates with stakeholders on data collection and performance improvement activities to enhance program efficiency

Required qualifications

• High school diploma with at least one year of post-high school medical education; associate degree preferred
• Minimum of one year of clinical oncology healthcare experience
• Comprehensive understanding of medical terminology
• Proficiency in computer databases and software, including Excel and Word; prior Epic experience preferred
• Demonstrated ability to function autonomously and effectively problem solve in a collaborative environment
