Job Summary
To support patient care coordination, the full-time Radiology Care Coordination Specialist will manage scheduling and referrals for specialty providers, track patient interventions and outcomes, and serve as a resource for patients and families, all while working remotely.
Key responsibilities
- Coordinates patient scheduling and referrals to specialty providers, ensuring clear communication and follow-up
- Tracks and documents patient interventions and outcomes, providing timely updates to care providers
- Collaborates with stakeholders on data collection and performance improvement activities to enhance program efficiency
Required qualifications
- High school diploma with at least one year of post-high school medical education; associate degree preferred
- Minimum of one year of clinical oncology healthcare experience
- Comprehensive understanding of medical terminology
- Proficiency in computer databases and software, including Excel and Word; prior Epic experience preferred
- Demonstrated ability to function autonomously and effectively problem solve in a collaborative environment
