Job Summary

A company is looking for a Recovery Navigator to coordinate care and support for patients and families in their treatment journey.

Key Responsibilities
  • Reinforce patient and family alignment to the treatment model and provide ongoing support
  • Conduct proactive and reactive outreach to enhance patient engagement and retention
  • Act as a liaison between patients and various care teams to ensure cohesive service delivery
Required Qualifications
  • Bachelor's degree in health sciences, business administration, communications, or a relevant field
  • Minimum of 2 years experience in a customer or patient success role
  • Knowledge of HIPAA policies and procedures
  • Proficiency with digital communication and software platforms
  • Bilingual in English and Spanish is highly preferred
