Job Summary

A company is looking for a Referral Care Coordinator - Contract to Hire.

Key Responsibilities

• Track incoming referrals and guide families into care quickly and compassionately
• Communicate with referring providers to build trust and provide updates
• Manage follow-ups and ensure no family slips through the cracks

Required Qualifications

• 2+ years in care coordination, medical administration, or customer success, preferably in pediatrics or behavioral health
• Experience with multiple systems (CRM, EHR, email, etc.)
• Able to work independently and meet daily targets
• Strong focus on follow-through and documentation
