Referral Coordination Team Lead

Job Summary

A company is looking for a Referral Coordination Team Lead to oversee referral processes and enhance partnerships in a remote setting.

Key Responsibilities

• Oversee the intake, processing, and tracking of incoming referrals
• Manage outreach efforts related to referrals and monitor engagement documentation
• Collaborate with Community Partnerships Managers to support external referral relationships

Required Qualifications

• 3-5+ years in healthcare operations, referral management, care coordination, or managed care
• 1-2+ years of management experience
• Experience with Medi-Cal, ECM, CalAIM, or community-based programs strongly preferred
• Strong process design and data analysis skills
• Experience with Google Workplace and/or Microsoft Office, specifically Google Sheets or Excel
