Referral Coordinator

Job Summary

A company is looking for a Referral Coordinator, Associate.

Key Responsibilities

• Manage incoming calls and coordinate referral inquiries across various communication methods
• Initiate and complete referral documentation while following up on interactions with referral sources and patients
• Provide exemplary customer service and remain current on department policies and procedures

Qualifications

• High school graduate required
• Minimum of 2 years of customer service and/or call center experience preferred
• Knowledge of Home Health and/or Hospice preferred
• Proficiency in typing and knowledge of Microsoft Office, Excel, and Adobe preferred
• Ability to work independently and demonstrate flexibility in work schedule
