Job Summary

A company is looking for a Referral Coordinator to manage medical referrals and ensure efficient patient care coordination.

Key Responsibilities

• Manage inbound and outbound medical referrals and maintain accurate documentation
• Review, triage, and follow up on referrals in the basket system
• Collaborate with providers, specialists, and patients to facilitate care coordination

Required Qualifications

• High School diploma or GED equivalent; post high school training or college preferred
• Minimum of 1 year experience in customer service and scheduling
• 6 months of Epic experience preferred
• Working knowledge of referrals and InBasket
• Medical terminology knowledge is essential
