Referral Coordinator

Job Summary

A company is looking for a Referral Coordinator.

Key Responsibilities

• Triaging, facilitating, and problem-solving all referrals and faxes received from external sources
• Processing incoming referrals according to established standards and entering data into the Epic EMR system
• Communicating with referring providers and other clinicians to ensure accurate and complete referral information

Required Qualifications

• Bachelor's degree with 2 years of experience in an ambulatory health care clinic, or
• Associate's degree with 4 years of experience in an ambulatory health care clinic, or
• High School Diploma with 6 years of experience in an ambulatory health care clinic
• Experience with data entry and attention to detail
• Ability to identify challenges and barriers in the referral process
