Job Summary

A company is looking for a Referral Coordinator to assist in connecting participants with referral resources and coordinating care.

Key Responsibilities

• Facilitates scheduling of participants and live transfers to the team
• Researches and contacts community resources and treatment providers based on clinical recommendations
• Documents all referrals and participant satisfaction with treatment providers and facilities

Required Qualifications

• Associate degree in Business, Wellness, Healthcare, or related field required
• Experience utilizing a case management platform
• Comfortable making numerous outbound phone calls daily
• Experience in Microsoft Office, including Word, Excel, PowerPoint, and Outlook
