Job Summary

A company is looking for a Referral Navigator to facilitate patient navigation through the healthcare system.

Key Responsibilities

• Manage inbound calls and requests for the Provider Finder Line program, ensuring timely follow-up with patients
• Maintain relationships with Emergency Departments and Urgent Care Centers to address patient needs for primary and specialty care
• Coordinate care by processing referrals and scheduling appointments in accordance with established guidelines

Required Qualifications

• High School graduate or equivalent
• One year of healthcare experience in patient registration or physician office
• Basic knowledge of insurance referrals, eligibility, and prior authorizations
• Experience with electronic medical record systems, preferably Epic
• Ability to maintain professional knowledge of changing insurance referral standards
