Referral Specialist

Job Summary

A company is looking for a Referral Specialist to manage patient enrollments from external referrals.

Key Responsibilities

• Conduct outbound calls and digital outreach to referred patients to drive new patient enrollments
• Document all incoming referrals and interactions in systems of record with timely updates
• Coordinate with health plan case managers and external healthcare providers to share updates on referrals

Required Qualifications

• Bachelor's degree in social work, healthcare, or a related field
• Experience working with various healthcare referral systems
• Ability to work independently and as part of a team
• Familiarity with Medicaid health plans is preferred
• Experience in a fast-paced and dynamic startup environment is a plus
