Job Summary

A company is looking for a Referral Specialist to manage patient enrollments from various healthcare providers.

Key Responsibilities

• Conduct outreach to referred patients to drive new enrollments and meet productivity requirements
• Document and process all incoming referrals and maintain accurate records
• Coordinate with health plan case managers and external providers to share updates and achieve enrollment goals

Required Qualifications

• Bachelor's degree in social work, healthcare, or a related field
• Experience with healthcare referral systems
• Familiarity with Medicaid health plans
• Ability to work remotely and independently
