Regional Care Coordinator

Job Summary

A company is looking for a Regional Care Coordinator to manage member experiences related to transportation services.

Key Responsibilities

• Coordinate member experiences by monitoring and confirming transportation trips
• Act as a liaison between health plan clients, members, and partners to ensure quality care
• Document escalations and action plans while maintaining compliance with KPIs and contract requirements

Required Qualifications

• High school diploma or G.E.D. equivalent
• 9 months of experience in a Customer Care Representative role or similar health care case management experience
• Proficient computer skills, including Microsoft Outlook, Word, and Excel
• Ability to work in a fast-paced environment and handle escalated situations
• Familiarity with medical terminology and patient care is preferred
