Regional Care Coordinator

Job Summary

Managing member experiences post-trip intake, the full-time Regional Care Coordinator will monitor and ensure successful transportation outcomes while working remotely in the Eastern Time Zone.

Key responsibilities

• Act as a liaison with health plan clients, members, and partners to coordinate care and support for identified members
• Monitor trips, confirm details, and resolve issues to ensure timely and accurate transportation for members
• Document escalations and action plans while adhering to policies and compliance with KPIs and contract requirements

Required qualifications

• High school diploma or G.E.D. equivalent
• 9 months of experience in a Customer Care Representative role or equivalent healthcare case management experience
• Proficient in Microsoft Outlook, Word, and Excel
• Ability to work in a fast-paced environment while maintaining confidentiality and accuracy
• Familiarity with medical terminology and patient care is preferred
