Job Summary
To enhance patient care coordination, the remote Regional Practice Manager will oversee the Patient Care Coordination team, ensuring high-quality patient support, efficient workflows, and achievement of key performance indicators while collaborating closely with clinical providers.
Key responsibilities
- Manage daily operations of the Patient Care Coordination team, including performance oversight and accountability
- Evaluate and improve PCC workflows, implementing process changes and maintaining documentation for the team
- Support clinical functions by coordinating patient intake, scheduling, and follow-up to facilitate successful visits
Required qualifications
- 3-5 years of experience in virtual healthcare operations, patient care coordination, or practice management
- Bachelor's Degree in Healthcare Administration or related field preferred; active MA Certification is a plus
- Proven track record of managing KPIs and driving team performance improvements
- Strong understanding of clinical workflows and patient support processes
- Excellent leadership and communication skills, with experience working cross-functionally
