Registered Dietitian Care Manager

Job Summary

A company is looking for a Care Manager, Health Management (Remote)-Registered Dietician (RD).

Key Responsibilities

• Provide care management and referrals based on clinical assessments and member needs
• Educate members on chronic health conditions and develop action plans for health management
• Monitor member progress and adjust care plans as necessary while advocating for members within the healthcare system

Required Qualifications

• At least 2 years of experience in healthcare, including 1 year in direct patient care or managed care
• Active and unrestricted license as an LVN, LPN, LCSW, LPC, or RD as required by state regulations
• Demonstrated knowledge of community resources
• Ability to work independently and adapt to diverse populations and situations
• Excellent time-management and prioritization skills
