Registered Dietitian Case Manager

Job Summary

A company is looking for a Part Time Registered Dietitian.

Key Responsibilities

• Provide comprehensive nutrition care and case management services to individuals with complex medical and social needs
• Develop individual care plans and follow up with patients to ensure coordinated care
• Document interventions, maintain privacy of member records, and assist in providing quarterly reports to clients

Required Qualifications

• Graduation from an accredited RN program and possession of a current United States RN license
• Bachelor's degree in nutrition, Dietetics, or a related field; Master's degree preferred
• Minimum of 3 years of clinical nutrition experience
• Prior experience in case management or coordinating wellness programs
• Basic computer literacy and proficiency in software applications like Microsoft Word and Excel
