Job Summary
A company is looking for a Health Coach - Wellness.
Key Responsibilities
- Assist members by completing health assessments and implementing care plans to improve health and wellness
- Provide telephonic support and coaching for members on wellness care plans and programs
- Monitor wellness plans and member outcomes, providing recommendations to reduce health risks
Required Qualifications
- Associate's degree and 2-4 years of related experience
- Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, or Master's in Behavioral Health required
- Health Coach Certification is required
- Strong preference for candidates with Registered Dietitian licensure
