Job Summary

A company is looking for a Clinical Health Coach to support and educate members with chronic conditions in achieving their health goals.

Key Responsibilities

• Manage a panel of high-risk members to improve health and manage chronic conditions using an innovative coaching framework
• Establish relationships with members through various communication methods including app, email, and telephone
• Support members in managing their conditions by responding to clinical triggers and assisting with the development of clinical content

Required Qualifications

• Registered Dietitian (RD) Certification required
• Bachelor's degree or higher
• 5+ years of experience in health coaching, health education, nutrition, or behavioral health
• Health Coach Certification preferred
• Bilingual in English/Spanish preferred
