Registered Home Infusion Pharmacist

Job Summary

A company is looking for a Home Infusion Pharmacist (PT).

Key Responsibilities

• Review and evaluate medication orders for appropriateness based on patient factors and medical history
• Prepare and dispense specialty infusion products while adhering to regulations and organizational policies
• Provide patient and caregiver consultations on the safe use of infusion therapies and facilitate training for new programs

Required Qualifications

• Bachelor's degree in pharmacy or Pharm D
• Current, unrestricted licensure as a Registered Pharmacist in the state of residence
• 1-3 years of experience in a hemophilia treatment center or similar setting providing direct patient care
• Knowledge of infusion/specialty pharmacy medication order systems and infusion procedures
• Additional training in an accredited Pharmacy Residency program preferred
