Job Summary

Seeking a dedicated Registered Nurse California, the full-time remote RN will manage patient care for seriously ill patients through telemedicine, ensuring adherence to care plans and coordinating with interdisciplinary teams.

Key responsibilities

• Manage assigned patients, aligning care team visit frequencies with patient needs and acuity levels
• Communicate with patients via phone and video, providing care and education based on ongoing assessments
• Facilitate care coordination activities, including medication management and collaboration with primary care providers

Required qualifications

• Active, unrestricted California Registered Nurse (RN) license; willingness to obtain licensure in other states
• 1+ year of experience in palliative or hospice care
• 1+ year of professional nursing experience within the last three years
• General computer skills and proficiency in MS Office and internet usage
• Availability to work Monday through Friday, 8:30am-5:00pm Pacific Time, with potential for evenings and weekends
