Registered Nurse Care Coordinator

Job Summary

Supporting diverse client populations, the full-time Registered Nurse Care Coordinator will provide compassionate, client-centered virtual nursing support, including assessments, care plan development, and resource navigation in a fully remote environment.

Key Responsibilities

• Complete intakes and nursing assessments to understand client needs and develop care coordination plans
• Support clients and families through education, coaching, and motivational interviewing to ensure understanding of care plans and self-management needs
• Coordinate communication with care team members and providers to enhance continuity of care and address client preferences

Required Qualifications

• Registered Nurse (RN), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN) in good standing
• Minimum 3 years of nursing experience in acute care, community care, or related settings
• Experience working with elderly populations and individuals with complex needs is preferred
• Proficiency in English; French fluency is strongly preferred
• Comfortable using technology, including electronic documentation platforms and Microsoft 365 applications
