Registered Nurse Care Coordinator

Job Summary

Providing essential support in the coordination of workers' compensation patients, the full-time Registered Nurse Care Coordinator will manage patient documentation, facilitate communication between providers and case managers, and ensure timely authorization of services while working remotely.

Key responsibilities

• Manages completion of care coordination tasks and ensures proper documentation is maintained for each patient
• Facilitates communication of evaluations, orders, and progress notes between healthcare providers and the nursing department
• Coordinates with Nurse Case Managers and adjusters to obtain necessary authorizations for continued patient care

Required qualifications

• Registered Nursing degree (R.N., L.P.N., or M.A.) with a minimum of two years of experience in an acute setting, home care, or medical office
• Knowledge of care management processes and the ability to navigate healthcare documentation systems
• Experience in customer service with a focus on building high levels of satisfaction
• Proficient in computer navigation and operation for documentation and communication
• Ability to work independently and collaboratively in a remote environment
