Job Summary

Leading a team of Telephonic Care Managers, the full-time Registered Nurse Care Management Manager will provide operational leadership for a remote 24/7 care coordination program focused on supporting patients during critical transitions in care following significant healthcare events.

Key responsibilities

• Oversee daily operations of the Home First Center, ensuring high-quality care coordination services and continuous coverage
• Provide leadership and clinical guidance to Telephonic Care Managers, fostering a culture of collaboration and patient-centered care
• Lead quality improvement initiatives aimed at enhancing patient experience and optimizing care transitions across the continuum

Required qualifications

• Registered Nurse (RN) with active licensure and a Bachelor's Degree in Nursing; Master's Degree preferred
• Five (5) years of progressive experience in care management or related clinical leadership roles
• Prior supervisory or management experience preferred
• CCM, ACM, or equivalent certification preferred
• Experience leading integrated care management teams and knowledge of population health and value-based care is advantageous
