Job Summary

A company is looking for a Nurse Care Manager.

Key Responsibilities

• Manage patients' specialized needs and develop comprehensive care management plans
• Coordinate patient care delivery with healthcare providers and community services
• Monitor and evaluate the effectiveness of case management plans and advocate for patients

Required Qualifications

• Current Registered Nurse license is required; Bachelor's of Science in Nursing preferred
• Certified Case Manager (CCM) credential preferred
• 3+ years of clinical practice in a healthcare setting highly preferred
• Prior experience with electronic health records or health registries required
• Microsoft Office skills, including Excel, Word, PowerPoint, and Outlook required
