Registered Nurse Care Manager

Job Summary

A company is looking for a Telephonic After Hours Nurse Care Manager (WEEKENDS).

Key Responsibilities

• Handle inbound communications from patients regarding symptoms or care concerns
• Consult and coordinate with team members to assess and implement patient care plans
• Provide health information and advice to callers while documenting member data in the medical record system

Required Qualifications

• Registered Nurse (RN) with current licensure in assigned state(s)
• Bachelor's degree, at minimum
• Minimum 3+ years of recent clinical experience
• Experience in a call center or triage position preferred
• Ability to work flexible weekend hours with a minimum of 32 hours per month
