Job Summary

A company is looking for a Telephonic Care Manager (RN) - HCBS Command Team.

Key Responsibilities

• Coordinate care and health education for Health Plan members through telephonic collaboration
• Conduct comprehensive assessments and develop individualized care plans to address members' needs
• Document all activities in the care management tracking system and identify trends for improvement

Required Qualifications, Training, and Education

• Minimum of 2 years of experience in a clinical setting and case management nursing
• BSN preferred
• Registered Nurse (RN) license required
• Case management certification or approved clinical certification preferred
• Private, secure designated workspace required in the home office setting
