Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager Transitions of Care.

Key Responsibilities

• Engage members and their support systems through assessments and care planning to address health needs
• Coordinate care and facilitate communication among interdisciplinary care teams and community resources
• Monitor member satisfaction and healthcare resource utilization while documenting care coordination activities

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or Bachelor's degree in a healthcare field, or equivalent experience
• Advanced degree associated with clinical licensure is preferred
• Minimum of three years of experience in nursing, social work, counseling, or related healthcare fields
• Three years of Medicaid and/or Medicare managed care experience is preferred
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor is required
