Job Summary

A company is looking for a Registered Nurse Care Manager.

Key Responsibilities

• Provide chronic disease management support and patient education for value-based care contracts
• Conduct comprehensive telephonic assessments to identify patient health needs
• Collaborate with an interdisciplinary care team to coordinate patient-centered care

Required Qualifications

• Registered Nurse licensure required prior to hire date
• Case Manager credential from the American Case Management Association (ACMA) preferred
• BLS Provider certification preferred
• Diploma from an accredited school/college of nursing required
• 3-5 years of experience in care management or related fields preferred
