Job Summary

A company is looking for a Senior RN Care Manager - Remote.

Key Responsibilities

• Manage patient caseloads, conduct assessments, and guide outreach and care team interventions
• Serve as the primary contact for patients in specialty care and support quality improvement initiatives
• Develop, implement, and monitor patient care plans, ensuring continuity of care and compliance with safety programs

Required Qualifications

• Bachelor's degree in Nursing or equivalent education/work experience
• Registered Nurse License or Compact State Licensure
• 5 years of clinical experience as a Registered Nurse (RN)
• Must be eligible to work in the United States without visa sponsorship
