Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager RN Mom and Baby.

Key Responsibilities

• Engage members and their support systems through assessments and care coordination
• Develop and update individualized care plans in collaboration with the interdisciplinary care team
• Facilitate communication and coordination among healthcare providers and community resources

Required Qualifications, Training, and Education

• Nursing degree from an accredited nursing program is required
• A minimum of three (3) years of experience in nursing is required
• Three (3) years of Medicaid and/or Medicare managed care experience is preferred
• Three (3) years prior maternity or NICU experience is preferred
• Current unrestricted clinical license in state of practice as a Registered Nurse is required
