Registered Nurse Care Manager

Job Summary

To support a growing care management team, the full-time Registered Nurse Care Manager will ensure timely care management, coordinate transitions of care, and empower members towards optimal wellness while working remotely and conducting fieldwork in various locations.

Key responsibilities

• Perform telephonic outreach and face-to-face assessments to develop and implement member-specific care plans
• Provide targeted health education and collaborate with care teams to facilitate safe transitions and prevent re-admissions
• Engage with interdisciplinary teams to improve member outcomes and maintain documentation according to regulatory standards

Required qualifications

• Current unrestricted Registered Nurse license in the state of residence
• Bachelor's degree or relevant equivalent experience; preferred in nursing
• Minimum of 5 years of relevant clinical experience; home care or case management experience preferred
• National certification in Case Management is desirable
• Proficiency in a second language is desirable
