Registered Nurse Care Manager

Job Summary

Providing coordination of care for SNP members, the full-time Registered Nurse Care Manager will establish and maintain care plans, collaborate with interdisciplinary teams, and ensure quality management of health services in a remote setting.

Key responsibilities

• Conduct intake assessments and develop individualized care plans for members
• Act as a liaison for the Interdisciplinary Care Team and manage member self-referrals to care management programs
• Ensure compliance with CMS SNP Model of Care expectations and assist in quarterly reporting of case management processes

Required qualifications

• Current Registered Nurse license issued by the state or multi-state license through the enhanced Nurse Licensure Compact (eNLC)
• Three years of healthcare clinical experience
• One year of Care Management, Case Management, or Population Health experience
• Knowledge of Medicare Advantage and NCQA accreditation standards
• Ability to perform virtual face-to-face visits to assist members in coordination of care needs
