Job Summary

Providing exceptional care and support for chronic health conditions, the full-time Remote Registered Nurse Care Manager will manage patient outreach, create personalized action plans, and coordinate with clinical team members to enhance patient engagement and experience.

Key responsibilities

• Provide disease management and health education to patients
• Support goal setting and create personalized action plans for managing chronic conditions
• Coordinate with clinical team members to ensure an exceptional patient experience

Required qualifications

• 2+ years of experience as a Registered Nurse
• 2+ years of experience in patient-facing or customer-facing roles
• Compact state license required; additional licensing may be needed
• Bilingual (Spanish speaking) is a plus
• Experience in telehealth or healthcare startup environment preferred
