Registered Nurse Care Manager

Job Summary

To support the health plan's medical management team, the full-time Registered Nurse Care Manager will coordinate care for SNP members, establish care plans, and collaborate with interdisciplinary teams, all while working remotely.

Key responsibilities

• Conduct intake assessments and develop individualized care plans for members
• Act as a liaison between members, caregivers, and healthcare providers while managing transitions of care
• Utilize NCQA standards to oversee care management processes and ensure quality care delivery

Required qualifications

• Current Registered Nurse license issued by the state or multi-state RN license through the eNLC
• Three years of healthcare clinical experience
• One year of experience in Care Management, Case Management, or Population Health
• Experience managing Medicare, Medicaid, or SNP populations is preferred
• Bachelor's Degree in Nursing, Associate of Science in Nursing Degree, or Diploma is preferred
