Registered Nurse Care Manager

Job Summary

As a full-time Telephonic Care Manager working remotely, the candidate will manage patient transitions in care following significant healthcare events through comprehensive assessments, individualized care planning, and coordination with healthcare providers and community resources.

Key responsibilities:

• Conduct comprehensive assessments and develop individualized care plans to address patient needs and barriers to care
• Coordinate transitions across various care settings and arrange necessary follow-up services to support patient recovery
• Advocate for patients and connect them with community resources, ensuring their preferences and goals are reflected in care planning

Required qualifications:

• Registered Nurse (RN) with active licensure or Master's Degree in Social Work (MSW) with LSW or LCSW
• Five (5) years of experience in care management, discharge planning, or related clinical experience
• Bachelor's Degree in Nursing preferred for RN candidates
• Certification as a Case Manager (CCM or ACM) preferred for RN candidates
• Knowledge of post-acute care services and healthcare delivery systems
