Job Summary

Providing clinical care management services, the full-time Registered Nurse Care Manager (RN) will coordinate care for eligible patients remotely, focusing on improving health outcomes and self-care abilities while managing a caseload and collaborating with healthcare teams.

Key responsibilities

• Utilize Motivational Interviewing techniques to engage patients in managing chronic conditions
• Develop and implement care plans to enhance healthcare outcomes and facilitate wellness
• Collaborate with healthcare providers to ensure effective patient-centered care plans

Required qualifications

• Associate's Degree in Nursing (required)
• RN compact license or Multi-State License (required)
• 2-3 years of experience in acute care, home health, or case management
• Case Management certification (preferred)
• Bachelor's Degree in Nursing (BSN) (preferred)
